We’ll review how the Practice Support Program (PSP) can empower (and fund) you to lead QI projects\, as well as how to use Health Data Coalition (HDC) tools to support your work. This session serves as a follow-up to our recent ‘Hypertension in Primary Care’ education session and will showcase a local hypertension QI project that demonstrates how to improve care with practice data.
