The project focuses on planning, implementing, and evaluating a pain and symptom management service for active cancer clinic patients in Kamloops. The project will link with PCN planning to identify accessible, comprehensive, and long-term primary care options for high priority oncology patients with the goal of shifting primary care delivery from specialists in acute care clinics (i.e., cancer clinic) to community, as well as exploring cancer screening options for unattached patients. Lastly, the project is excited to pursue collaborative relationships with the health authority and local, regional and/or provincial organizations to plan for/enhance access to cancer supportive care for patients in Kamloops and the Thompson Region.

- Improvements in physician and other health provider coordination, flow of care and communication
