Shared Care Committee
Managing mental health care for patients with moderate complexity or atypical psychosocial concerns presents significant challenges for providers, especially when these patients don’t fit typical diagnostic categories. There is a growing need for patient-centered improvements within the system, particularly for those whose needs fall outside the scope of current service streams. This project will address those gaps by engaging diverse stakeholders in identifying prevalent issues in mental health care delivery, as well as conducting process and patient journey mapping to understand the current state and pinpoint critical gaps.
The project will focus on exploring the development of a robust toolkit and referral pathway to enhance communication and collaboration among providers. This toolkit will be designed to streamline mental health care processes, improve coordination, and ultimately ensure that patients with moderate complexity and psychosocial concerns receive the specialized and personalized care they need.

By conducting a thorough needs assessment and exploring practical, patient-centered solutions, we aim to craft a detailed, actionable plan to refine and expand mental health care services in the Kamloops region. This will ensure a more responsive, effective care system for those who currently face barriers to receiving appropriate care with the goal of enhancing service delivery and improving overall patient outcomes across the region.
