Joining us in PCN Health Hub (within North Shore UPCC), you will work with the interdisciplinary Primary Care Network team supporting patients from across the Thompson Region.
Please note that this position will require a valid divers license, and travel to rural communities surrounding Kamloops. Re- deployment may be required based on operational needs.
In this role, you will conduct comprehensive, evidence-based assessments of clients’ physical status and functional abilities, identifying their needs, goals, and expectations. You will develop and implement personalized treatment plans to improve functional independence, promote health, and prevent injury or disability. Your expertise in physiotherapy will be pivotal in delivering high-quality care and supporting clients in achieving their optimal physical performance. You’ll offer guidance and direction to rehabilitation assistants, ensuring that specific care needs are met and functions are transferred appropriately for optimal client outcomes
